In the last year I seem to have developed a side stitch, but not like the one you get running. This one is along the rib cage to the right of the sternum (as in "this is my right arm" side). It is maybe 3 inches around from the sternum. It happens only during sustained maximal efforts. It seems to also involve the upper ab in that area. It is like a knife being stuck in, very sharp and very painful.

In one race I tried massaging the area, and maybe it helped or maybe we slowed down, but it went away. Today I am still sore from yesterday if I push on that area, which is new as I don't remember a lingering pain before. It doesn't hurt if I don't squish it. It hurts when you try to stick your fingers a little under the ribs. The left side is completely fine.

What on earth could this be and how can I escape it? Have I torn something? It's not an immediate pain, but one that seems to come after a few minutes (3-5) of going all out.

Knobby Toes:

Have an evaluation by your doctor. It could be simply costrocondritis an inflamation to the sternal ribmargin that may benefit from local ultrasound thereapy, hot compresses and a course of anti-inflamatory therapy. It also could be something more serious such as angina (heart pain) as it has been occuring when you push yourself hard on the bike during a race and is eases up when you back off. A physical evaluation, possibly an EKG and stress test are in order.
