Have a structural leg length diff of 7mm. It's corrected easily in my road shoes, but the shims not working great for the MTB shoes.

I am using the crank bros peds with the pre-made shims from bikefit.com. Problem is the shimmed cleat is super loose in the pedal and I'm pulling out of the pedals a lot.

Has anyone else solved this issue?

I have not had the opportunity to assess different cleat shims to give you an informed recommendation. Check with a quality bike shop that has Bike-Fit certified technitians and they may be able to help you find a workable cleat. Another option is to get a shorter crank on the side of your shorter leg. This is more expensive but may be a more effective alternative for you.
